I want to help my father apply for Veterans Assistance. What sort of documents will I need to include with his application?

Since you’re trying to help you father, you will want to fill out VA form 21-0845 so that they are authorized to discuss the application with you. There are a number of documents that you will need to turn in with your application. Typically, fully complete applications get through the process faster than uncompleted. Here's what you'll need to include:

  • Discharge/separation papers (also known as DD Form 214)
  • Copy of Marriage Certificate
  • Copy of current Social Security Award Letter
  • Net worth information, including bank accounts, CD’s, trusts, stocks, bonds, annuities, etc.
  • Proof of insurance premiums, medications, medical bills or any other medical expenses that are not reimbursed by Medicare or insurance
  • Proof of all income
  • Physician statement
  • Banking information for Direct Deposit of monthly payments
  • List of all doctors and hospitals visited in last year
